The Sauquoit Valley School District has been selected to report Student Weight Status category information for the 2016-2017 school year. Student Weight Status Category information will be collected from the private / school physicals submitted to the schools during the previous school year 2015-2016. Our school will submit the information from the 2015-2016 health physicals for students that were in grades UPK, K, 2, 4, 7 and 10 during 2015-2016. This information will be collected and tallied by the School Nurses and submitted to the New York State Department of Health. Information will be reported using the Health Commerce System, the Department of Health’s secure internet site.
You may wish not to have your child’s weight status group information included as part of the New York State Health Department survey this year.
To opt-out please visit the Sauquoit Valley Central School District web site and print out an Opt-Out Form to be completed and return to your child’s School Nurse by December 8, 2016. (Click here to download the Opt-Out form)

New York State will be implementing the following Meningococcal vaccine requirements starting in the 2016 – 2017 school year.
The only exception to this would be if the student received the 1st dose of the vaccine after their 16th birthday.
