  • BMI Opt Out Form

    Our school district was selected to take part in a survey by the New York State Department of Health for the 2014-2015 school year. We will be reporting to New York State Department of Health information about our students’ weight status groups from the previous 2013-2014 health certificate/appraisal forms. This will include students that were in grades UPK, K, 2, 4, 7, and 10 during 2013-2014 school year. Only summary information is sent no names and no information about individual students are sent. However, you may choose to have your child’s information excluded from this survey report. 
    The information sent to the New York State Department of Health will help health officials develop programs that make it easier for children to be healthier. 
    If you do not wish to have your child’s weight status group information included as part of the Health Department’s survey this year, please complete this opt-out form and return to your child’s school nurse by December 8, 2014.

  • Disclosure of Annual Professional Performance Review 
    New York State law allows parents and legal guardians of a student to request the composite effectiveness scores and final quality ratings of teachers to which the student is assigned for the current school year. To request this information about your child’s current teacher please complete this form and submit it to the Main Office of your student’s school building. It is the obligation of the District to verify all information provided in this request.

  • Parent Portal Information
    We have received several requests from parents about the Parent Portal.  Below are two documents that should help answer any questions.  The first is a form to request access to your child's records through SchoolTools.  The second is step-by-step directions for using the SchoolTool Parent Portal.  You will find a link to the Parent Portal at the top of every webpage on our school's website.
