It is highly recommended that you SUBMIT YOUR CHILDS ANNUAL EXAM AND IMMUNIZATION RECORD EACH AND EVERY YEAR. This allows your child to remain in school, participate in physical education, fulfill necessary mandated screenings and meet immunization requirements.In addition, ALL students who want to participate in our sports program will require a recent physical on file. The exam must be less than one year old.ALL reports can be mailed, emailed or faxed to the appropriate building nurse noted above.New York Education Law requires medical examinations of school children at specific grade levels. A health certificate (proof of physical exam) shall be furnished by each pupil in the public schools in grades UPK, K, 1st, 3rd, 5th, 7th, 9th and 11th as well as all new students. An examination of any child may be required by local school authorities at any time in their discretion to promote the educational interests of such child. The physical must be given within 12 months of school entrance.
Tdap Alert
The State of New York requires that all students who turn 11 years old have a Tdap immunization.If they are not immunized within 14 days after their 11th birthday, they will not be allowed to attend school.Acceptable proof of tetanus,diptheria and pertussis (Tdap) is a signed note from the doctor or the clinic where the shot was given. An appointment is sometimes not necessary, and they will also fax it to us at 315-839-6390 (middle school fax)Any further questions, please call the middle school nurse, Mrs. Evans at 315-839-6372.
